STUDENT ACTIVITY RELEASE OF LIABILITY

In consideration of being allowed to participate in any way in an on-campus or off-campus Huntington University event, program, activity (hereinafter referred to as “Activity”), I the undersigned, do willingly and voluntarily acknowledge, appreciate, and agree to the following:

1. I acknowledge that within the course and scope of my activities, I may be exposed to hazards or risks that may result in my illness, personal injury, or death, and I understand and appreciate the nature of such hazards and risks. In consideration of being permitted to participate in the Activity, I hereby accept all risk to my health, including any injury or death and property that may occur while I am acting within the course and scope of the Activity as an intern or otherwise participating in the Activity. To the best of my knowledge, I can fully participate in this Activity.
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation. 3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard or concern during my presence or participation, I will remove myself from participation and bring such to the attention of the University immediately.
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS Huntington University, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the activity ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property associated with my presence or participation, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
5. I further understand and acknowledge that HU is not an insurer of my personal safety or property. I UNDERSTAND THAT THE UNIVERSITY WILL NOT BE RESPONSIBLE FOR ANY MEDICAL COSTS ASSOCIATED WITH ANY INJURY I MAY SUSTAIN. I understand that the University requires me to obtain adequate health and accident insurance to cover any personal injury that may be sustained during the Activity or during transportation to and from the Activity. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Supervision, Housing, and Protection Protocols
I understand that University staff will not share sleeping rooms with minors. Overnight housing will be in residence hall rooms hosted by vetted and trained Huntington University students. Program staff will supervise participants through regular check-ins, curfews, and adherence to Huntington University’s youth protection policies.

Transportation Consent
I consent to my child’s transportation in University-owned, rented, or contracted vehicles, driven by authorized staff or volunteers, for program activities.

Mandatory Reporting
I understand that Huntington University employees are mandated reporters under Indiana law and must report suspected abuse or neglect of a minor to state authorities.

Confidentiality of Records (FERPA/HIPAA)
Health and emergency contact information provided will be maintained confidentially by Huntington University and shared only with staff or medical providers as needed for the safety and care of my child, consistent with FERPA/HIPAA and applicable state law.
